Becoming a New Patient

To become a patient at our office, the following forms will need to be completed.  Once the forms are received, they will be reviewed and you will be contacted within 24-48 hours to set up your first appointment.  Our office will not give healthcare advice over the phone until your child is established as a patient.

Please remember to contact your insurance company or case worker, if needed, to change your primary care provider. This must be done prior to your child's first appointment. 


Please provide the following along with your new patient paperwork: 

A copy of your insurance card

Up to date vaccination records

Any specialty medical records for continuation of care


After you fill out your new patient paperwork, you may drop it off at our office or fax it to us at 910-321-7337.


**For patients other than newborns: We will need a medical release form filled out 

to retrieve medical records from your previous primary doctor


**Please note, if you no show to your new patient appointment, you will no longer be considered a patient at our office and will not be able to be rescheduled.

Directions to set up patient portal account:

  1. click the link above for the patient portal - web or mobile version.
  2. click "View your health record" at the top of the page
  3. type your provider's name in the "Find your practice to view your Health Record" field or enter the office code you received via email.
  4. enter the patient's name and date of birth
  5. enter the code that was sent to the phone number on file for the patient - (your phone number must be up to date on file)
  6. you should now have access to the patient portal
